Disabled Placards/Plate
 
In order to apply for a placard, obtain a Handicap/Disabled form from the County Treasurer or print option #14 from Forms and take it to your doctor for completion.  Return the signed application to the Treasurer’s Office or mail it to The Ellis County Treasurer, PO Box 520, Hays, KS 67601-0520 along with a $.50 mail fee.  The placard will then be mailed to you.  Or you may drop the application by the office and be given the placard at that time, no fee.
 
When applying for a plate, obtain a Handicap/Disabled form from the County Treasurer or print option #14 from Forms and take it to your doctor for completion.  Request for a disabled license plate must be made in person.  Present the license plate registered to your vehicle, your current registration, and proof of current Kansas liability insurance.  Your current registration plate will then be surrendered in exchange for a disabled plate. The fee is $3.50.
